Thank you for your interest in Shepherd of the Hills’ 2010 Baby Dedication Service, which will take place on Friday, June 25 at 7:00 pm.
Attached please find the required application for participation. At the bottom of page one, we provide the dates and times of the Baby Dedication Classes offered. Parent(s) must attend one of the three classes in order to participate in the Baby Dedication service. Please place a check mark next to the class you will be attending. Only parent(s) are asked to attend the class.
Please return your completed application to Donna Tharp, either via regular mail, emailed to dtharp@theshepherd.org or drop them off at the main office (Monday through Friday, between 9:00 am and 5:00 pm), no later than June 11, 2010.
